Task:

Design a simple and reusable customer feedback form template that informal businesses in Sierra Leone (e.g., tailors, food vendors, barbers, mechanics, etc.) can use to collect feedback from their customers. The goal is to help these businesses understand their customers' experience and improve their service delivery.

Deliverables:

• A printable version (PDF or Word) and a digital version (Google Form or Typeform link) of the feedback form

• The form should include:

• Must include instructions for how the business can edit and reuse the form (e.g., change name/logo or create a new link)

• Include a sample filled-in version for context

Submission Requirements:

• The form must be designed with actual informal businesses in Sierra Leone in mind — please select and reference a real business (e.g., a local tailor, cookery, garage) when designing the sample.

• The tone and questions must be friendly, respectful, and easy to understand.

• Mobile accessibility is important — test it on a phone.

• Upload both the printable and digital versions, along with a short write-up (max 1 page) explaining how it works and how the business can use it.

Judging Criteria:

• Clarity & Simplicity: Is the form easy to fill out, even for customers with limited literacy or tech experience?

• Usefulness: Does the form collect meaningful feedback that can help the business improve?

• Reusability: Can other businesses easily duplicate or adapt this form?

• Presentation: Is it visually neat, clean, and welcoming?

Important Note:

We will be handing over these forms to real businesses for actual use. Please work with or reference a real informal business in Sierra Leone for your submission.
